Caregivers are adult family members or other individuals who
are unpaid providers of in-home and community care to
older adults. They often don't see themselves as caregivers
as they are simply doing what a family member or friend
would do. yet the services they provide are invaluable to
the person they care for, and to the community at large.

between two people that the court creates to protect persons
who are incapacitated and cannot handle their own affairs.
When an adult is incapable of managing their own affairs or
is incapable of managing their own affairs or is competent
but ill, a conservator protects them from physical danger
and improper treatment, and/or protect their estate from
exploitation. Under Minnesota laws, conservatorships and
guardianships are very similar, except that guardianships

A Health Care Directive or a Advance Directive helps
you keep control over health care decisions that are
important to you. It is a document in which you give
instructions about health care if, in the future, you cannot
speak. You can give someone you name (an agent or proxy) the
power to make health care decisions for you. You also can
give instructions about the kinds of health care you do or
do not want. In the case of a temporary or permanent illness
or injury, this document will work in those situations as
well. This is very important document and requires some
thought, so here are some resources to help you! Talk with
your doctor as you consider your health care needs and this
important decision making process

Health Education
Health Education services are those that provide
information and advice about your health. They can include
information on your body, disease management, medication,
treatment options, and prevention measures. The education
can be provided by many different organizations like
hospitals, senior centers, colleges, social services,
trained volunteers, disease specific organizations, or
extension services. Contact your local senior center or;

Nursing Home
There are a lot of questions and concerns when thinking
about a nursing home as a health care option. A nursing home
is a place of residence for people who require constant
medical care, but at a lower level than a hospital. Usually
the residents are elderly, but a nursing home can include
mentally or physically ill persons as well. A Patient Review
Instruction is required by a doctor to enter a nursing home
care. When looking at nursing homes it is wise to visit
several before you make your decision.
