Health Assessments

A senior health assessment is used to evaluate your current health and in the process may help diagnose any current or future problems. It can improve health, lengthen life, reduce the need for acute nursing care, and keep older adults from the Emergency Room. Some questions that might be asked during an initial assessment include: How are you feeling? What illness or surgeries have you had? How's your life going on a daily basis? Do you need help? Contact your physician for more information.

Health Care Directive

A Health Care Directive or a Advance Directive helps you keep control over health care decisions that are important to you. It is a document in which you give instructions about health care if, in the future, you cannot speak. You can give someone you name (an agent or proxy) the power to make health care decisions for you. You also can give instructions about the kinds of health care you do or do not want. In the case of a temporary or permanent illness or injury, this document will work in those situations as well. This is very important document and requires some thought, so here are some resources to help you! Nursing Home

There are a lot of questions and concerns when thinking about a nursing home as a health care option. A nursing home is a place of residence for people who require constant medical care, but at a lower level than a hospital. Usually the residents are elderly, but a nursing home can include mentally or physically ill persons as well. A Patient Review Instruction is required by a doctor to enter a nursing home care. When looking at nursing homes it is wise to visit several before you make your decision.
